CASE MANAGER - 2127

Requires a degree from an accredited nursing school (BSN preferred) or other professional degree with appropriate clinical and/or project management experience. Certification in Case Management preferred.

  • CLINICAL CARE MANAGEMENT
    • Full-time, Days, 40hrs/week
      The Case Manager ensures that the patient care progression from acute care episode through post discharge is quality-driven, efficient, and effective. The Case Manager and other care team members work collaboratively to ensure patient needs are met and care delivery is coordinated across the continuum. The Case Manager seeks the clinical expertise of the supervising Manager to help resolve patient care issues and enhance patient transition/discharge plan as needed. The Case Manager assists the patient/family to navigate the health care system and communicate with the interdisciplinary care team providers through the episode of care until the planned level of functional health is achieved by the patient at discharge.

UTILIZATION MANAGER - 2004

Requires graduation from a state-accredited school of professional nursing (BSN preferred) or other professional degree with appropriate clinical and/or project management experience. Must be licensed to practice as a registered nurse in the state of North Carolina by the North Carolina Board of Nursing. CCM preferred.

SKILLS REQUIRED: UTILIZATION MANAGEMENT

  • CLINICAL CARE MANAGEMENT

    The Utilization Manager ensures that the patient care progression from acute care episode through post discharge is quality-driven, efficient, and effective. The Utilization Manager and other care team members work collaboratively to ensure patient needs are met and care delivery is coordinated across the continuum. The Utilization Manager completes admission, continued stay, and discharge reviews in accordance with federal regulations and the hospital utilization management plan. In addition, the Utilization Manager is responsible for ensuring revenue is optimized by reconciling physician orders, bed billing type, and medical necessity. This may include delivering notifications to patients directly. Interface is completed verbally, via emails, Canopy tasks, or other electronic communication and via telephone.

    • 40hrs/week
